Please complete the Check Request Form and include all required receipts, invoices, or supporting documentation. Providing complete information helps us process your request as quickly as possible.

Before You Submit
 

Please make sure:

• Your expense has been approved by the MRE PTA.

• The Check Request Form is completed in full.

• All applicable receipts or invoices are included.

• The requested amount matches the supporting documentation.

• Your contact information is included in case we have any questions.

Mini-Grant & Teacher Wish List Purchases
 

If you are requesting reimbursement for an approved PTA Mini-Grant or Teacher Wish List purchase, please submit a Check Request Form along with your receipts for the approved items.
